Your first visit is a comprehensive consultation focused on understanding your health and concerns. Dr. Sabet will review your medical history and prior records, perform a detailed examination, and work with you to discuss your goals, lab findings, and next steps in your care.

Includes:
• Detailed review of prior records before your appointment
• Visit with Dr. Sabet which will include review of medical history, lifestyle, symptoms and examination
• Ordering of comprehensive labs and imaging as needed
• Telehealth follow-up visit with a discussion of diagnoses and treatment options if needed
• Communication with your primary are physician

Please note: The initial consultation does not include prescriptions for new medications, direct messaging access, or additional phone consultations outside of scheduled visits. These services are only available through our membership plan.

After the initial consultation, Dr. Sabet will either recommend membership or determine ongoing disease management is not required at that time. Should ongoing disease management be recommended and the membership program is declined by the patient, the patient assumes risk and the responsibility to find care elsewhere.

For patients with a diagnosed rheumatologic condition, our membership plan provides ongoing, comprehensive care and direct access to Dr. Sabet.

Includes:
• Regular follow-up visits (up to 6 per year) with minimal wait times and priority scheduling with additional visits available at a discounted fee of $125
• Ordering of comprehensive labs and imaging as needed
• Use of insurance for labs, imaging and medications
• Direct messaging with Dr. Sabet during clinic business hours to stay connected between visits for questions, updates or concerns
• Assistance with medication refills and prior authorizations to keep your treatment on track
• Personalized, long-term, rheumatologic care that is tailored to your health needs and goals
• Coordination of care with your primary care physician and other medical team

Membership Terms:
Membership requires a minimum three-month commitment. A 30-day written notice is required for cancellation.

• Patients with abnormal lab results or findings that require observation but no active treatment
